Head Start Enrollment Requirements

In order to apply a child for enrollment in the Head Start program, you will need to have the following information with you when you apply:

  • Legal Birth Certificate
  • Certificate of Immunization (Pink cards not acceptable)
  • Social Security Cards for applicant and all household members
  • Proof of 2003 Income or Notarized Statement of Income
    (Examples: 2002 W-2 Form or Tax Return)
  • Medicaid Card (if child has one)
  • Current phone number, address, Driver's license number and Social Security number of two (2) emergency contacts
  • Food Stamp Verification
  • Documentation of child's special needs or disabilities from a Health Service or Social Service Professional
  • The name of your Caseworker or Facilitator (if applicable)

All information must be present before application will be taken to ensure that the application can be processed in a timely fashion.
