Head Start
Enrollment Requirements
In order to apply
a child for enrollment in the Head Start program, you will need to have
the following information with you when you apply:
- Legal Birth Certificate
- Certificate of
Immunization (Pink cards not acceptable)
- Social Security
Cards for applicant and all household members
- Proof of 2003 Income
or Notarized Statement of Income
(Examples: 2002 W-2 Form or Tax Return)
- Medicaid Card (if
child has one)
- Current phone number,
address, Driver's license number and Social Security number of two (2)
emergency contacts
- Food Stamp Verification
- Documentation of
child's special needs or disabilities from a Health Service or Social
Service Professional
- The name of your
Caseworker or Facilitator (if applicable)
All information must
be present before application will be taken to ensure that the application
can be processed in a timely fashion.
